Gadaa Care AFH (APD) provides person-centered residential care for the elderly. We offer a safe, supportive, and home-like environment with 24-hour care, including assistance with activities of daily living, medication management, and individualized support to promote independence and well-being.

Our nurse-led home prioritizes dignity, choice, and quality of life, while addressing each resident’s physical, emotional, and social needs. We work collaboratively with families, case managers, and interdisciplinary care teams to ensure consistent, compassionate, and high-quality care.

• 5 private, comfortable rooms
• Nutritious home-cooked meals and snacks
• Housekeeping, laundry, and linen services (excluding dry cleaning)
• Assistance with scheduling medical appointments and coordinating with healthcare providers

• 24-hour care, supervision, and nurse oversight

• Ongoing health monitoring, vital signs checks, charting, and medication management

• Personal care, including assistance with activities of daily living (ADLs) and incontinence support

• Diabetic care, including blood sugar monitoring and insulin administration

• Wound care, oxygen therapy, respiratory support, and chronic condition management

• Specialized dementia, Alzheimer’s, and cognitive support

• Hospice and respite care services • Support with physical, occupational, and speech therapy (PT, OT, ST)

• Mobility and fall prevention assistance • Seizure monitoring and emergency response planning

• Engaging activities and social events to promote social interaction and well-being Specialized cares:

• Diabetic care, including insulin administration

• Specialized dementia and Alzheimer’s care

• Ostomy/colostomy care. 

• Oxygen therapy & respiratory support – monitoring and assisting with oxygen use, nebulizers, and breathing treatments

• Seizure monitoring and emergency response planning
